1.Scope: According to PCP version 9 para 161(a) a duly completed "Post registration changes request form"(CDM-PRC-FORM) shall be submitted.
Issue:A PRC form has not been submitted.
2.Scope: The types of post registration changes are not consistent among the submitted documents.
Issue: The monitoring report refers to a request for deviation for the period 1/10/2014 to 6/12/2014, however no PRC documents have been submitted except the PRC validation report for PRC which is submitted as additional documents.
Please note that if a PRC is requested a PRC request must be submitted with all related documents.
