Submission incomplete:
1: Scope: The monitoring report does not contain a description of the equipment used to monitor each parameter including details on accuracy class and calibration information (frequency, dates of calibration and validity) as specified by the monitoring methodology and the monitoring plan as per PS version 09.0 paragraph 248 (b).
The verification report (CAR C3) and the monitoring report (p. 31 ) state that the Gas chromatograph (GC) was installed in March 2013. Details of all GCs used during this monitoring period shall be provided.

2: Scope: The verification and certification report does not list each parameter required by the monitoring plan and does not provide an statement on how the DOE verified the information flow for these parameters including the values in the monitoring reports as per VVS version 09.0 paragraphs 393 and 409 (e).
Issue: The monitoring plan states that the parameter W_steam,CH4,y shall be monitored at least every three months. However, the emission reduction spreadsheet indicates a lower monitoring frequency. For example, the sampling dates for well OW-701 are 13/03/2013, 06/01/2013, 15/09/2013 and 21/12/2013. In addition, the samples grouped by quarter do not always fall within that quarter. For example, under Q1 (Jan-Mar) a sample for OW-726 is included from 03/12/2013 which is outside of the indicated quarter. Please clarify how the sampling frequency of the registered monitoring plan was complied with.

3: Scope: The verification and certification report does not provide an assessment on whether the calibration of measuring equipments was conducted at a frequency specified by the applied methodology, the standardized baseline and/or the monitoring plan as per VVS version 09.0 paragraph 400.
Issue: Section E.7 of the verification and certification report refers to the calibration of a conveyor belt balance, drying oven and analytical balance. This seems inconsistent with the types of meters used in the project activity.
In the monitoring report the gas chromatograph has the serical number 12131 and was installed in March 2013. The Gas chromatograph in the verification and certification report has the serial number 020462.
The monitoring report mentions that the diesel meter (Sr. Nr. 29B) was not duly calibrated and the emission reductions sheet takes this into account. Section E.7 of the verification and certification report asserts that all meters were duly calibrated.
The DOE is requested to address these inconsistencies.

4: Scope: The verification and certification report does not provide a statement on whether post-registration changes to the registered PDD have been approved by the Board or will be submitted together with the request for issuance as per VVS version 09.0 paragraph 409 (f).
Issue: No reference is made in section E.4 (and its subsections) to the change in monitoring plan (approved 12/03/2012) nor the change in the PDD (approved 09/02/2015)
