Submission incomplete:
1.Scope: A monitoring report using the form and guidelines for completing the monitoring report form (CDM-MR-FORM) has not been submitted with a request for issuance as required in the completeness check checklist for requests for issuance in line with PCP version 9 para 222.
Issue: The monitoring report contains a blank page (page 32). Kindly clarify.

2.Scope: The monitoring period throughout the documentation is not consistent.
Issue: There is an inconsistency in the date of the monitoring period. The verification and certification report refer to monitoring period 15/09/2014-14/07/2015 whereas the applicable monitoring period is 15/07/2015-25/03/2016. Kindly clarify.

3.Scope: The cross-referencing and versioning within and between the document are not correct and accurate.
Issue: There is an inconsistency in the cross-referencing of the standardized baseline. The monitoring report and verification and certification report refer to methodology AM0010 and standardized baseline ASB0001 whereas the applicable PDD (v. 12 dated 19/02/2009) only refers to methodology AM0010. Kindly clarify.
