Issue: 1. The monitoring report and verification/certification report refer to post-registration changes: temporary deviation whereas a request for post-registration changes and corresponding documents are not submitted along with the issuance request.


2.Scope:
Issue 2: The issuance request is submitted under VVS version 2 which is no longer valid since 7 October 2021.

3.Scope: The cross-referencing and versioning within and between the document are not correct and accurate.
Issue: 3. There is an inconsistency in the cross-referencing of the monitoring report. The verification and certification report on pages 11, 48 and 51 refers to monitoring report version 3 dated 14/07/2021, on pages 9 and 66 to version 4 dated 22/10/2021 and on pages 10 and 93 to version 5 dated 07/06/2021 whereas the applicable monitoring report is version 6 dated 25/03/2022.
4. There is an inconsistency in the cross-referencing of the monitoring report form version. The verification and certification report on pages 2, 9 and 67 refers to monitoring report form version 8, which is no longer valid, whereas the submitted monitoring report is version 9.
